These prices are effective as of  January 9, 2019 but thereafter subject to change without notice
The goods and services shown below are those we can provide to our customers.  You may choose only the items you desire.  However, any funeral
arrangements you select will include a charge for our  basic services and overhead.  If legal or other requirements mean you must buy any items you 
did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected.

BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF---------------------------------------------------------------------------------------------------- $1,300.00
Our fee for the basic services of funeral director and staff includes, but is not limited to, staff to respond to the initial
request for service, conducting funeral or memorial arrangement conference with the family or responsible party,  
preparation and filing of necessary authorizations and permits; recording vital statistics; preparation and placement
of obituary notices; staff assistance prior to; during and following the funeral, including coordination with those 
providing other portions of the funeral (i.e. cemetery, crematory, and others). Also included in this charge are over-
head expenses such as insurance, utilities, secretarial and administrative cost, equipment and inventory expenses.
Register book and memorial folders are included.  This fee for our basic services and overhead will be added to the 
total cost of the funeral arrangements you select.  (This fee is already included in or charges for direct cremations,
immediate burials, and forwarding or receiving of remains.)

EMBALMING----------------------------------------------------------------------------------------------------------------------------------------  $     500.00
Except in certain cases, embalming is not required by law.  Embalming may be necessary, however, if you select certain 
 funeral arrangements, such as a funeral with viewing.  If you do not want embalming, you usually have the right 
to choose an arrangement that does not require you to pay for it, such as a direct cremation or immediate burial.

Forwarding of Remains to Another Facility------------------------------------------------------------------------------------------------------------  $ 1,295.00
This charge includes removal of remains, basic services of funeral director and staff, necessary authorizations, embalming,
local transportation to the airport. This charge does not include the service of staff or the use of facilities for any visitation or ceremonies,
transportation, shipping container of merchandise prior to forwarding of the remains.

This charge includes limited services of staff, sheltering of remains, and transportation of remains to the cemetery within a
50 mile radius.  The charge does not include visitation rites, ceremonies, or transportation beyond 50 miles.

This charge includes: removal of remains within a 75 mile radius, local transportation to the cemetery, necessary authorizations
dressing, casketing, equipment and basic services of staff.  This does not include embalming, or use of a facility for any visitation or
ceremony, public or private.  This charge does not include casket or any other merchandise such as outer burial container, clothing,
register book of acknowledgement cards prior to burial.  Outer burial container may be required at some cemeteries.

Our charge for a direct cremation (without ceremony) includes: basic services of funeral director and staff, a proportionate
share of overhead cost; local removal of remains; transportation to crematory; necessary authorizations; state permit,
crematory fees and sales tax on alternative container.
If you want to arrange a direct cremation, you can use an alternative container.  Alternative containers encase the body
and can be made of materials like fiberboard or composition materials (with or without an outer covering). 
The containers we provide are corrugated cardboard boxes.
